Healthier Living Colorado Workshops...
A 6-week Chronic Disease Self Management Program

Diabetes/Pre-Diabetes Workshop runs from September 9th to October 14th 2014 at Community Partnership
in Divide on Wednesdays, 5:00-7:30PM.

Have you or someone you love been diagnosed with Diabetes or Pre-Diabetes? The workshop uses an evidence-based approach that will benefit your health through active self management. Participants will improve their health and increase their quality of life by finding practical ways to deal with better nutrition and exercise choices, understand new treatment options, and learn better ways to talk with your doctor and family about your health. Workshop leaders teach concrete steps such as goal setting, creating action plans, relaxation techniques, healthy eating and communication skills. Moving for Better Balance -
Better Balance Equals Fewer Falls

Moving for Better Balance is a twelve week falls prevention program using the principles and movements of Tai Chi designed to help adults 60 years and older improve their balance and increase their confidence in doing everyday activities.
