2) Drop off form in person to our offices located at 800 Research Drive, Woodland Park, CO Suite #100

3) Mail form to:
Teller County Community Development Services
P O Box 1886, Woodland Park, CO 80866

The fee of $10.00 will be collected prior to release of the information.
Accepted forms of payment are check, cash or credit card (credit card service fee is .99¢)
